Brain Health Toolkit

When nursing staff are screening for delirium, it is important to have evidence-based actions for them to take to properly address delirium. These strategies are generally beneficial for the prevention of delirium as well. These resources can be used by nurses, CNAs, volunteers, 1 on 1s, or family members/caregivers. 

 

When possible and if needed, consider if you can find a sponsor to support starting your kits and keeping them stocked. 

Toolkit Content

The content of each toolkit should be specifically tailored to the unit based on resources that are already available. In some cases, having consults for music therapy, recreation therapy, occupational therapy, and physical therapy may also be an option. 

1. Playdough or model magic clay (for best hygiene practices use single use packets).

2. Ear plugs and face masks (if appropriate) to promote sleep

3. Adult coloring sheets, crayons, colored pencils, markers

4. Paint with water sheets and paint brushes

5. Music players to listen to the radio or favorite tunes

6. Doodle boards that can be wiped down and reused

7. Seated exercise guided activities for individuals who cannot get up and walk

8. Guided meditation and relaxation activities for those need relaxation support before bed

9. Decaffeinated herbal tea

10. Magazines & Newspapers

11. Reminiscent kits about generalizable topics

12. Informational sheets for family members or caregivers of individuals who have delirium

13. Informational sheets for nursing staff about how to handle challenging behaviors 

 

NOTE: for all items consider single use items or those which can easily be wiped down with a sanitary wipe.
